Well done Ann that's progress indeed & encouraging. I recently took a flight to Slovenia. I was determined not to let paruresis deter me. Like you I had to try hard to manage in certain situations. I came seriously unstuck just once with a high level of urgency and just couldn't manage to go in the toilet of a department store, I didn't feel comfortable confiding in the two women with me so I endured a lot of physical pain and held on for the next 4 hours till we arrived back where we were staying. That is one of my worst fears one of failure, having a medium/high level of urgency and not being able to go I'm trying not to be too hard on myself as I am pushing the boundaries and making progress. Having said that I am finding that sometimes my paruresis affects me more than others, very often when I'm least expecting it. I don't know if you've experienced this? Well done to you again on your climb.
