It is not clear from your posting whether your son is affected by parcopresis i.e. cannot defecate at school, or paruresis ie cannot urinate at school. The former is less of an issue, whereas the latter is a real problem at school.
A common cause of the problem is bullying in the school toilets. Any sign of shyness, i.e. using a cubicle, can get picked on by bullies, and this makes life miserable for the lad concerned: he needs to pee each day, and he knows that the bullies know this. So he is on a hair trigger in the toilet, listening for any foosteps in case it will be them.
Another unpleasant form of bullying is more genitally focussed e.g mocking a boy who is a late developer for not being as advanced genitally as his peers.
Do you have a male role model that he would be willing to speak to?
Alternatively could you point him to this site, telling him that he can post anonymously?
Or would he talk to me (as an ex-sufferer who has helped several hundred men and women) on the phone; point out that the phone is effectively an anonymous medium; I dont need to know his name, nor where he lives etc.
