Thanks for sharing your touching story. Remember, you're never alone when in contact with this site.
As Andrew said - at this point, catheters are the best option. Have you considered them? I have used them and found them to be a god sent when on a 9 hour stag night! They are quick and easy to use aswell.
Imagine you are in your workplace, you would be able to take say 3 caths with you each day. Once you are used to being able to go when ever you want, you should find that you can start to go without using them. You know you will always have a failsafe if you cant go naturally, therefore, anxiety is reduced.

In the meantime, you should start de-sensing by yourself. If you want some help, then let us know exactly what stage you are at ie. when can you/cannot go and in what situations?
I do hope that you will be able to get into another job, what did you do peviously?
